Abstract

Abstract New Daily Persistent Headache (NDPH) is a primary headache disorder defined by a sudden, clearly remembered onset followed by continuous, unremitting headache persisting for at least three months. Despite well-defined diagnostic criteria, NDPH remains mechanistically unexplained and therapeutically refractory in many patients. We propose that NDPH is best understood not as a slowly progressive sensitization disorder, but as a rapid phase transition into a stable high-gain state within coupled nociceptive, autonomic, and immune-modulated neural networks.
